Job Duties:
Plan, direct and coordinate activities of a multi-disciplinary team in a community-based day care center to provide social services to aged adults and adults with chronic medical, cognitive or behavior health conditions (review participants’ medical and financial conditions to assess needs for mental and occupational therapies and counseling, nursing and healthcare, social and living programs; conduct psychosocial evaluation with GDS and MMSE to develop IPC; provide individual counseling using Motivational Interviewing and other tools; develop healthcare options to assist participants in coping with illness, mental distress and living barriers; maintain documentation and develop discharge plans); implement policies to ensure compliance with governmental and agency regulations.

Qualification Requirements:
- Master of Social Work awarded by an accredited university or college in the United States;
- 2 years’ prior on-job work experience as a social worker or director of social service program in an adult care facility or center;
- Possess certificate of training or course completion in TF-CBT, Motivational Interviewing and Cultural Competency.

Proffered Pay:
$70,990 / year (full-time)
