Mission

Fontana Adult Day Health Services is a Community Based Adult Service/Adult Day Health Care (CBAS/ADHC) program dedicated to promoting the physical and emotional well-being and dignity of adults 18 years or older with certain chronic medical, cognitive, or mental health conditions, or disabilities who are at risk of needing institutional care.

To help local community members stay mentally and physically active while reducing social isolation, improving health, maximizing independence, preventing hospitalization and premature nursing home placement, and providing support and relief to caregivers.

Are you eligible?

Medical Qualifications

To be admitted to Fontana Adult Day Health Care Services, the application should be frail elderly or functionally impaired adults over the age of 18 having all of the following conditions:

  • One or more chronic or post-acute medical, cognitive or mental health condition(s) identified by a health care provider

  • Limitation in the performance of two or more ADLs (Activities of Daily Living) and/or IADLs (Instrumental Activities of Daily Living).

  • Insufficient network of non-ADHC support services, including family or caregivers to maintain the individual in the community

  • High potential for medical, cognitive or mental health conditions that may lead to institutionalization and need for professional nursing, social, therapeutic and meal services.

How to enroll.

Follow these steps if you are interested in our services! Call us if you have any questions.

Step 1

Call (909) 901 - 3001 to confirm eligibility.

Step 2

Schedule a visit. Bring picture ID, Medi-Cal, Medicare, SSN, Health Plan ID and PCP contact information.

Step 3

Health & Physical Exam (within 3 months); TB Test (within 12 months).

Step 4

Waiting for authorization.

Step 5

Health plan 1x phone or face-to-face evaluation to verify medical eligibility.

Step 6

Three-day assessments. Transportation and meals provided.

Step 7

Sign Participation Agreement.

Step 8

Enrollment complete.
