Mission

Fontana Adult Day Health Services is a Community Based Adult Service/Adult Day Health Care (CBAS/ADHC) program dedicated to promoting the physical and emotional well-being and dignity of adults 18 years or older with certain chronic medical, cognitive, or mental health conditions, or disabilities who are at risk of needing institutional care.

Vision

To help San Bernardino community members stay mentally and physically active while reducing social isolation, improving health, maximizing independence, preventing hospitalization and premature nursing home placement, and providing support and relief to caregivers.

Are you eligible?

Medical Qualifications

To be admitted to Fontana Adult Day Health Care Services, the application should be frail elderly or functionally impaired adults over the age of 18 having all of the following conditions:

  • One or more chronic or post-acute medical, cognitive or mental health condition(s) identified by a health care provider

  • Limitation in the performance of two or more ADLs (Activities of Daily Living) and/or IADLs (Instrumental Activities of Daily Living).

  • Insufficient network of non-ADHC support services, including family or caregivers to maintain the individual in the community

  • High potential for medical, cognitive or mental health conditions that may lead to institutionalization and need for professional nursing, social, therapeutic and meal services.

Step 1

Please call (909) 901 - 3001 to confirm eligibility and schedule a visit.

How to enroll.

Step 2

Bring a copy of picture ID, Medi-Cal Card, Medicare Card, SSN card, Health Plan Membership ID and PCP contact information on your date of visit.

Step 3

Complete health & physical exam with PCP within 3 months of application, including a TB test exam within 12 months of application.

Step 4

Waiting for health plan authorization.

Step 5

Health plan schedule either phone or face-to-face evaluation to confirm medical qualifications.

Step 6

Schedule 3 days of assessments in the center, transportation and meals are provided on the dates of assessments.

Step 7

Sign the Participation Agreement and enrollment complete.
